There are three separate but related programs, each serving a different population

Access to Home for Heroes / Veterans
Program Overview
This program provides financial assistance to low- and moderate-income veterans with disabilities. Funds may be used for accessibility modifications, emergency home repairs, or to correct code violations in the veteran’s primary residence.
Eligible applicants may be homeowners or renters. Disabilities may be related to service-connected injuries, age, or health conditions.

Program Overview
The Access to Home for Medicaid Recipients Program is administered in partnership with the New York State Department of Health (DOH). It provides financial assistance to help Medicaid recipients with disabilities make their homes accessible.
The program supports independent living and aims to reduce long-term healthcare costs by preventing institutional placement.
RESTORE
Residential Emergency Services to Offer (Home) Repairs to the Elderly
Program Overview
The RESTORE program provides financial assistance to help senior homeowners address emergency repairs and code violations that threaten their health, safety, or ability to live independently at home.
These critical repairs help seniors remain safely housed and avoid displacement or institutional care.

Eligibility Requirements
Applicants must meet all of the following:
At or below 80% of Area Median Income (AMI), or
Up to 120% of AMI for eligible veterans certified by the U.S. Department of Veterans Affairs or the Department of Defense as entitled to disability benefits for a disability incurred during time of war
The assisted unit must be the applicant’s primary, permanent residence, whether owner-occupied or rental.
For rental units, the property owner must not already be legally required to make the requested accessibility improvements.

Submit RESTORE ApplicationI recently had an accessible bathroom renovation completed through the Access to Home Program. After nine months of challenges, especially when it came to showering, being able to use the new accessible shower has truly been life-changing.
Being able to do more on my own has made a tremendous difference, not only for me but also for my wife, who had to help me every day. We cannot fully express how much this means to us or how deeply grateful we are.
