Enhanced Multidisciplinary Team (E-MDT)

Protecting Older Adults Through Professional Collaboration

The Enhanced Multidisciplinary Team (E-MDT) is a network of professionals who work together to address complex cases involving older adults experiencing abuse, neglect, self-neglect, or financial exploitation.

The E-MDT combines the expertise of Adult Protective Services, Offices for the Aging, law enforcement, healthcare providers, legal professionals, mental health specialists, financial institutions, and other community partners. By sharing information and coordinating services, the team develops comprehensive plans that improve safety, preserve independence, and strengthen outcomes for vulnerable older adults.

Who Can Benefit?

The E-MDT serves older adults who may be experiencing:

• Physical, emotional, or sexual abuse
• Financial exploitation or scams
• Neglect or self-neglect
• Social isolation or unmet care needs
• Cognitive impairment that increases vulnerability
• Situations requiring coordinated intervention from multiple agencies

How the E-MDT Works

The E-MDT reviews referrals submitted by participating professional agencies. During confidential case review meetings, team members identify risks, recommend interventions, coordinate services, and connect individuals with available community resources.

Because the E-MDT is a professional case review team, it does not accept referrals directly from the public.

Eligible referrals are made by agencies such as:

• Adult Protective Services (APS)
• Office for the Aging (OFA)
• Law Enforcement
• Healthcare Providers
• Hospitals
• Financial Institutions
• Legal Service Providers
• Other participating community agencies

Hudson Valley E-MDT

BRIDGES serves as the Hudson Valley Hub for the Enhanced Multidisciplinary Team, coordinating regional collaboration in:

• Dutchess County
• Orange County
• Rockland County
• Sullivan County
• Ulster County

BRIDGES Hudson Valley E-MDT Coordinator facilitates team meetings, supports participating agencies, coordinates case reviews, and promotes collaboration to improve outcomes for older adults throughout the region.

If you believe an older adult is experiencing abuse, neglect, self-neglect, or financial exploitation, contact the appropriate agency in the county where they live. These organizations can assess the situation and determine whether an E-MDT referral is appropriate.
